Error with Upgrading to 2.14

I am attempting to upgrade from version 1.2 to 2.14
I have followed the upgrade instructions as provided but I am receiving the following error:
"Notice: Undefined index: cookiePrefix in /home/cityave/public_html/cmsAdmin/lib/init.php on line 377 Couldn't start session! 'Cannot send session cache limiter - headers already sent (output started at /home/cityave/public_html/cmsAdmin/lib/init.php:377)'!"
I have the permissions set as directed, and the server specs are
Linux Apache/2.2.13 (Unix)
PHP 4.4.9
MySQL 5.0.92

Hi edavis711,
My first recommendation is to restore your version to 1.2 (if you haven't already) and try the update again, making sure that all the components of 2.14 are uploaded and overwrite any pre-existing versions of those files.
When you login again you will see that message "Notice: Undefined index: cookiePrefix in /home/cityave/public_html/cmsAdmin/lib/init.php on line 377 Couldn't start session!" flash up for a moment but ti should continue with the upgrade and bring you to the login screen again.

I have tried what you recommended but I am still receiving that error.
I did notice that I do not get to the log in screen at all, just the error.

The best thing to do is to raise a second level support ticket so we can have a look directly at the server and find out what's happening.
