Form Issue

Hello,

I have a form here at this page: http://www.charlotteparent.com/photos/form.php

I have a category field that will let the user select the according category. I can select the drop down category and fields with no problem. The issue arises after I have uploaded my picture to the form. Once I upload the picture, the category drop down loses all the categories and I can no longer select the category. It is an issue because I want that to be a required field and I need the selected category to be recorded in database. Can you help me figure out how to fix that?
