Signing-up Twice with the Same User Info?

I've just opened a new CMSB web site and discovered a problem I've not seen before. Users are registering on the site multiple times - using the same data - without being prevented from doing so!

The site is set-up to use the user's email address as their username. I have checked the accounts table and both username and email address are unique fields. Even so, the same user can sign-up multiple times using the same email address over and over again!

When a user registers their account is automatically disabled. This allows the admin an opportunity to scrutinise the user before approving them and "un-disabling" the account. If there is a pause between registration and approval the users can get impatient and some have been known to register again - which is how this problem came to light.
