error from website membership user-signup form

I have built a fairly extensive form using the website membership user-signup as a base. It all works well, but I receive this message:
Notice: Undefined index: to in /home/americanwinds/public_html/cmsb/lib/common.php on line 1229 Notice: Undefined index: from in /home/americanwinds/public_html/cmsb/lib/common.php on line 1230
I see this is for the "from" and "to" fields, but I don't know where to find and/or set these.

Thanks for posting.
Are you using an email template with your signup form?
If you are, could you double check that in the template itself (ie: log into CMS Builder and look at the email template there) has the "to" and "from" fields set.
If that's set and looks correct, could you post a copy of your form so I can review the code?

I believe I found my error! I had a bit of older code in the sending part of form. Updated this and it works fine now!
Had This:
// send message
@$emailHeaders['headers']['CC'] = "me@me.com";
$mailErrors = sendMessage($emailHeaders);
list($mailErrors, $fromEmail) = wsm_sendSignupEmail($userNum, $passwordText);
Changed to:
// send message
list($mailErrors, $fromEmail) = wsm_sendSignupEmail($userNum, $passwordText);
if ($mailErrors) {
